I acknowledge that I myself, and my dependent(s) have been given the opportunity to participate in the safe environment curriculum of the Diocese of Saint Cloud for the parish faith formation/Catholic School program. At this time I decline to participate in the Safe Environment curriculum for myself and/or my dependent(s).
